Hi, this is a case i have never faced before and could use some advice. Help!
Female, 80 y.o. , came to our outpatient facility, after 9 months of unsuccesfull physiotherapy, in another private practice, for a humeral head fracture ( 2 screws: close to greater and lesser tuberosity).
Patient presents with 15-20 degrees elevation and only a few degrees (0-5) in the glenohumeral joint.
The patient in the previous months -the previous PT practice- attended an aggressive physiotherapy protocol with resistance training and weights which were initiated around the first two to three weeks (cant tell).Patient experienced excruciating pain but continued therapy.
The patient now complains of muscle pain (triceps mainly) which restricts abductio, scaption, FF. The patient also experiences pain due to 'bone grinding; as she refers to the pain.
I have initiated a program comprising of deep friction, myofascial release for the entire area muscles, P and AROM, TENS, ultrasound. We are now on session 4 week 2. Pain has subsided from 8/10 to 5/10 and the patient can acheive painless scaption ROM 0-70 degrees only once or twice.
I would appreciate any advice on this situation.
